Informed Consent to Exercise Program
This Informed Consent to Exercise Program ("Consent") describes the nature, benefits, and risks of the fitness program provided by Focused Fitness Personal Training LLC, doing business as Muscle Mommy Coaching ("we," "us," "our"). By signing this Consent, checking a box that references it, or participating in any session, you confirm that you understand and agree to the terms below.
1. Purpose of the Program
You are enrolling in a coaching program that may include strength and resistance training, mobility and corrective exercise, conditioning, and guidance on training habits. The program is designed to help you build strength, improve body composition and movement quality, and support general fitness. It is tailored to your individual assessment and goals, particularly for women navigating perimenopause and postmenopause.
2. Description of the Activities
During sessions you may be asked to perform exercises using body weight, free weights, machines, resistance bands, and other equipment. Your coach will demonstrate movements, assign loads and progressions based on your movement screen, and monitor your form. You may train alongside other clients in a semi-private setting while following your own individual program.
3. Benefits
Regular, progressive strength training is widely recognized to support muscular strength, bone density, joint stability, balance, metabolic health, and overall quality of life. Individual results vary and no specific outcome is guaranteed.
4. Risks and Discomforts
You understand that exercise carries inherent risks, including muscle soreness, strains and sprains, joint or musculoskeletal injury, changes in heart rate and blood pressure, lightheadedness, fainting, and, in rare instances, serious cardiovascular events or injury. Your coach will take reasonable steps to reduce these risks through screening, instruction, and appropriate programming, but the risks cannot be eliminated entirely. Assumption of these risks is addressed in our Waiver.
5. Your Responsibilities
To participate safely, you agree to: complete our Health History honestly and completely; obtain your physician's clearance before beginning if you have any medical condition, are pregnant or postpartum, or have been advised to do so; disclose all injuries, illnesses, medications, and symptoms; follow your coach's instructions and use proper technique; and stop immediately and inform your coach if you experience pain, dizziness, chest discomfort, shortness of breath, or any other warning sign.
6. Not Medical Advice
Your coach is a certified fitness professional, not a physician, physical therapist, dietitian, or licensed medical provider. The program does not diagnose, treat, or cure any medical condition and is not a substitute for professional medical care. See our full Medical & Health Disclaimer.
7. Freedom to Withdraw
Your participation is voluntary. You may ask questions at any time, decline any exercise, and stop or withdraw from the program at any time. Withdrawal does not affect your right to any refund you may be entitled to under our Refund & Cancellation Policy.
8. Emergency Care
In the event of a medical emergency during a session, you authorize us to summon emergency medical services and provide reasonable assistance. You are responsible for any resulting medical costs.
9. Acknowledgment and Consent
You confirm that you have read and understood this Consent, that your questions have been answered to your satisfaction, and that you knowingly and voluntarily consent to participate in the exercise program. If you are under 18, a parent or legal guardian must review and consent on your behalf.
